I use Qpsmtpd in conjuction with qmail. You can write your own plugins in perl for authentication and other SMTP functions.

It's a little more resource intensive than C based smtp servers, but I was able to alter one of the existing auth plugins for the way I had vopmail installed and customize my setup quickly. It's easy to customize, and easy to setup.
