Well personally I find that too many hash marks are distracting. They kind of attract my eye but not to whatever they might try to emphasize but rather to the hash marks themselves!

I normally use just one; two can appear when I want to block-comment a block of code and its comments.

But I do use Smart::Comments and I do hope these are not obnoxious by default ;)
