Maybe you've got an autohandler/dhandler that ignores errors? Restart apache? http://search.cpan.org/~drolsky/HTML-Mason-1.40/lib/HTML/Mason/Admin.pod#ERROR_REPORTING_AND_EXCEPTIONS
