I had a brief look at SMS::Send and AFAIS one needs to specify a driver like SMS::Send::Twilio - which is a separate CPAN module - to make it work.

So you don't need to patch SMS::Send but SMS::Send::Twilio and upload it as a new module SMS::Send::Plivo.
