I see two more areas of possibility:

1) if the DB is on a different system than where the perl code runs, then you have two system clocks in play.

2) it is possible in a DB for the IDs to be allocated in a different order than the inserts are issued for other reasons e.g. self-locking or even certain DB constraints.
