Is it a problem at all in other companies?

I suppose so, though I don't really consider it to be a problem. My job involves a lot of management-dictated project development. Sometimes they, and myself, don't think of the best ways to do something at the time of the request. I consider changes and feature requests to be part of the project development life-cycle.
