A while ago, I was doing some refactoring where I needed a listing of all the subs and sub calls in a large program, which seems like almost what you want. I did a huge amount of searching through the Monestary. Here are some previous nodes that might be of help to you:

• Tabulate sub defs, sub calls in Perl code
  This seems to be the closes to what you want. It lists "(a) what subroutines are defined in each source file, and (b) what subroutine calls are issued from within each defined subroutine (or from the "main" portion of a given file)". However, I have had some problems with it complaining of syntax errors when there were none. YMMV.
  
  
• Generate a Graph-ical call tree for your *.pm perl modules
  This generates a tree of all your sub calls. However, it only registers calls that are made with an ampersand: ie. &sub(@args). A reply toward the bottom of the thread suggests first running your code through B::Deparse because it outputs sub calls in this format.
