I agree, I'm very guilty of replying without OK'ing the node.

I think a possible solution would be that, when replying to a node that has not been OK'd, there should be a checkbox after the entry that basically allows the replier to OK a node at the same time they reply to it, assuming that the monk in question has this power. It would be once less click, and something not as easily forgotten. Sure, this can have some small abuse potental (no worse that one being able to self-frontpage nodes), but can easily be dealt with by higher-ups.
