What you're describing is the layout on metacpan.org. I just tried three arbitrary modules to check this hadn't changed:

The TOC reflects the module's POD. If that is poorly written, or absent, you won't get what you're expecting.

You actually wrote cpan.org. Did you mean that or metacpan.org? If cpan.org, can you provide some example URLs so that we can see what you're looking at.
