I prefer to keep such things as simple as possible unless there is a significant performance advantage. I'd prefer something like:

or possibly 
printf qq{<input type="checkbox" name="fieldName" value="1" %s/>\n}, $
+fieldValues{fieldName} ? 'checked="checked"' : '';
