Welcome to the monastery, I am not familiar with DBIx::Dump. But I read from the documentation that this module is simply reading a DataBase and dumps the data into an excel sheet or CSV file.
From the documentation:
DBIx::Dump allows you to easily dump database data, retrieved using DBI, into a variety of formats including Excel, CSV, etc...
So I assume your question is how to add a title on the excel sheet.
So why not first dump the data into an excel sheet, and then simply use another Perl module e.g. Spreadsheet::WriteExcel to modify the excel sheet based on your desire.
I do not think that can be more complicated than that.
