XS bindings to the C/C++ libs in DCMTK would be amazing; having the building blocks to easily replicate or expand their utilities. Master list of tool/lib groups. Probably the most important toolset. A test DICOM SCP.

This would probably be a huge amount of work but you asked. :P If this full kit were available it would be a game changer for Perl use in medical IT.
