I believe this is a problem in your SQL syntax, not your Perl. In short, your SQL statement should be:

INSERT INTO Questions(Question,Answer_A,
                               Answer_B,
                               Answer_C,
                               Answer_D,
                               Answer_E,
                               Correct_Answer)
VALUES(?,?,?,?,?,?,?)
