Instead of the statement my $DSN...... use:
my ( $server_name, $database, $user_id, $password ) =qw ( SERVER DATABASE USER PASSWORD );

my $DSN = "driver={SQL Server}; Server=$server_name; Database=$database; UID=$user_id; PWD=$password";
