Yep, i had to make a few changes myself - your problem is not inside index.cgi, it is inside MyVars.pm: 
use CGI; # don't you mean use CGI qw(:standard); here?

You are accessing CGI::a() and others as if you did. Add ':standard' or ':all' and you should be fine.
