For a quick solution, you might look at the
Class::DBI
module. Provided your table setup is simple enough and form isn't overly complex either, this solution should be just the right one for you.
Unfortunately, I couldn't verify this on cpan.org (as it simply didn't respond for me ;/), but there should be another module named something like Class-DBI-Form. Which allows seamless integration of database tables and html forms.
Update: oh, here, I actually found a link to the module (which I had saved some time ago). The module name is in fact
Class::DBI::FromCGI
I've found the accompanying documentation to be fairly thorough, so hopefully you won't have a lot of trouble piecing things together. If indeed you have, just ask here and someone will be there to help :)
