Creating the software is a piece of cake compared to training people to use it, maintaining it and extending it for new/modified uses over its lifetime. I'd imagine the lifetime would be fairly long (~10 years?) for an institution like a hospital. Software companies are -- or should be :-) -- built to handle situations like this. A lone consultant isn't.
