This should get you started:
#!/usr/bin/perl -T use strict; use warnings; use CGI qw/:standard/; use CGI::Carp qw/fatalsToBrowser/; my $cgi = new CGI; $cgi->autoEscape(); print $cgi->header(); print $cgi->start_html(); print $cgi->start_form(); my %label_names = ( '1' => 'Whole Genome', '2' => 'Whole Genome with MIPS Subsets', '3' => "Specify a MIPS Subset\n" . $cgi->scrolling_list( 'scrolly', ['eenie','meenie','minie','moe'], ['eenie','moe'],1,'true'), '4' => 'Custom Gene Subset Enter =&gt ' . $cgi->textfield('random_name'), ); print $cgi->radio_group(-name=>'sets', -values=>[sort {$a<=>$b} keys %label_names], -default=>'1', -linebreak=>'true', -labels=>\%label_names); print $cgi->end_form(); print $cgi->end_html();

In order to get CGI to add the extra form fields in I needed to use autoEscape() so that the html would not be escaped (see autoEscape() in CGI docs).

On a side note, I would personally use HTML::Template (or other templating system) so my I could better seperate code from HTML.
