I don't see the problem. At least this form lets you fill whatever you want in the state field, instead of having you choose from a list of US states. You could probably use the "département" or whatever it takes for the postal service to find your address. If there's really nothing, you could fill in "DOES NOT APPLY, YOU INSENSITIVE CLOD!!!".

As for the zip code/postal code, that's just how different countries choose to call a similar concept, so again just fill in whatever it takes to identify your address to the postal service (and perhaps your credit card company).
