His point is that 

$foo->bar($x)->baz($y)

can be either 
$foo->bar($x);
$foo->baz($y);           # same object

[download]
or 
my $bar = $foo->bar($x);
          $bar->baz($y); # different object

[download]
which is the ambiguity he's talking about.
