Via VNC.

Seriously - my team is about 3200km from me. So we hook up via VNC over VPN to a common desktop somewhere (a user set up especially for code reviews which has no other permissions on that machine or network), bring the code up in our favourite syntax-highlighting editor, with a terminal open for using "diff -u", and go over the pieces while on the phone.
