That means that Apache::SSI is not getting loaded or used. Try commenting out

AddHandler server-parsed .shtml

(the non-Perl SSI) from your web server's configuration file and adding

<Files *.shtml>
SetHandler perl-script
PerlHandler Apache::SSI
</Files>

[download]

(the mod_perl SSI) in its place.
