You might need to use Win32::OLE to automate the application which created the file. I note in the Object Browser for MS Word, for example, that you can access the DocumentProperties collection (such as "Comments", "Category", or "Keywords") via VBA, and what you can access via VBA you can access using Perl and Win32::OLE.
