My Toronto YAPC buddies Richard Dice and Michael Graham demonstrated CGI::Application::Framework which sounded like it took care of everything you're talking about when it was presented at YAPC 2005 in Toronto. I just checked, and it is available on CPAN, although the warning does state that it's alpha quality software.

The docs look pretty complete, so have a look anyway.
