I think part of the problem is that you are trusting things to work properly, without testing them. It would be a good idea to test for values or errors, just to make sure you're not geting odd value somewhere, that is messing you up. You could check values and die if they're not what you think, or simply log everything and look at them later on to make sure they're what you were expecting.
