For most tokens I am content with [doc://] and [mod://], but once in awhile I find a keyword that doesn't work.

You can link to map and grep, but not for or foreach. For those you need something like this: [doc://perlsyn#Foreach-Loops-for-foreach|foreach] (foreach)
