Please read writeup formatting tips, especially the part about adding <code> tags around your code. That's not readable.

On the upside, you're using a fairly well-known script; NMS formmail. The README file for that script states that if you add a (hidden) field to your form named "redirect" with a URL value, the script will redirect to that URL (containing the page you want to show in response). In other words, you won't need to change the code at all, just add a field to the HTML form.

You might want to get the current version of that script by the way.
