This looks a lot like CGI ERROR RELATING TO MYSQL.

Again the questions: Why do you use Win32::ODBC instead of DBI with DBD::mysql? The latter is probably easier to install and use.

How exactly did you install Win32::ODBC?

Does it work if you execute it locally, ie not as CGI trough the web server?
