The solution I discovered is this... 

$contents =~ s/(<IMG\s.*?>)/&addalt($1)/sige;

sub addalt {

   if ($returned!~m/\salt/){
      $returned=$returned.'alt=""';
   }
   return $returned;
}

The key I discovered was the "e" switch on the regex.
