I recommend using CGI.pm, and I think most people on this site will agree. A typical response page would look like this:
#!/usr/bin/perl
use CGI qw(:standard);
use strict;

my $cgi = CGI->new;
print $cgi->header;
if ($cgi->param("name") ne "") { print "Welcome " . $cgi->param("name"
+) }

[download]

The HTML form to invoke this would be: 
<form method=post action=yourscript.pl>
<input type=text name=name>
<input type=submit name=submit>
</form>
