Are you asking how to upload this to a webserver? Wouldn't you be better simply using HTML, since your perl code just prints headers then an html page? I'd suggest taking a look at the CGI module Ovid's CGI Course - Resurrected and Updated! and Web Programming from the tutorials section of this site.
