*foo{NAME} and *foo{PACKAGE} are documented in http://perldoc.perl.org/perlmod.html#Symbol-Tables. They are somewhat different from the others.

*foo{FORMAT} is only available from 5.7.0 on. *foo{FILEHANDLE} is a deprecated version of *foo{IO}.
