    The solution to this problem is in two parts:

    • Create HTML to display the input fields that you want
    • Write Perl that outputs that HTML

    Which stage are you having problems with? And what do you have so far?

    I should point out that the first stage is really off-topic for this site.

    Not a perlish answer, but maybe The Coolest DHTML / JavaScript Calendar is something for you...
