PDF::Template is what's on CPAN for templatized PDFs. It's a bit old and not well-supported - in fact, it's looking for a new maintainer.
My criteria for good software:
- Does it work?
- Can someone else come in, make a change, and be reasonably certain no bugs were introduced?
