Here's a little coaching, coachd23: the two above and several others were found by asking Google; Super Search would have produced at least those two results. The point is that it's not only the Monks who offer help here; the monastery itself can often help you find what you need without recourse to personal appeals to the Monks.

And if these don't satisfy, there's always the old-fashioned way: create the pdf (or html) with appropriate blanks and let the users print it out; then fill it in.

    thanks. can you direct me on how to post a question to the monastery gates? i find it bizarrely difficult to understand how to do that. i'm sure i'm missing something simple. thanks.

      1. Create an account here (ie, register)
      2. Read docs, faqs, etc; then contribute good nodes
      3. Acquire enough XP to attain the Brotherly rank at which one is permitted to consider a node for Gates

      The ability to place a node on the Gates is reserved to Monks with some modest seniority (eg, experience).

      That, however, is not a warstopper for your use of the site. The title of any question you post as a root node (in Seekers of Perl Wisdom) will be visible to all -- for a time, in Newest Nodes and for an even longer (yay, even 'indeterminate') period in SOPW.
