It occurred when trying to commit the post Re^3: threads and Net::OpenSSH.

In contrast to previous occasions, it only happened once. Backing up in my browser and retrying, it worked at the second attempt.

Previously, it would take many (dozens) of attempts. Occasionally leading to the many unintentional duplicates situation.

    I believe this is fixed now as well. (A relatively new PerlMonks module from about a decade ago finally got added to the list of modules to just always be loaded instead of each of the many places that use it having to remember to make sure that the module is loaded.)

      I did see it a couple of times in the part week or so, but backing up and retrying (once) fixed it both times, which was a considerable improvement on previously.

      As I never had a reproducible scenario, there's nothing I can do to directly verify the fix, but I'll be sure to report back here if I see any further occurrence.
