Give this a try, http://bixsolutions.net/forum/post-58.html#pid58. Basically, DBD::mysql has to be built with native OS X headers from source. Not very difficult to do for the most part but it can bite you.

    Hi dhoss, you made my day! Compiling from the source = no problem at all. I always believed, there is not much difference between cpan and making it from the source, basically cpan downloads archive, unpack, Makefile, make, make test and make install. But there is difference, now it is working!
