I tried it, but same problem. I already had a header printed anyway, but I didn't include it in that snippet I posted.

This it turning into a whole "thing." Seems like something isn't working correctly, and it might not be my code. There is a discussion about it at http://www.aota.net/forums/showthread.php?s=&postid=86603 and so far, the solution seems to be a Perl script to do what the form should be doing.
