For what puts one user off CPAN (the utility rather than the archive), see Re^3: Installing Template::Toolkit on Windows. When I asked a similar question - how would you do CPAN differently - at a London PM social, I was introduced to semantic versioning (http://www.semver.org) as something that would be an improvement.
