I made an edit to the bottom of the post with the problem/solution. The documentation said to use: 
$self->params('affiliates')

[download]
when it should have been: 
$self->params->{affiliates}

[download]
I got HTML::FormHandler's IRC channel and informed them and they have now corrected the documentation.
