    Thanks. I've included them in my copy, but I didn't realise when I posted the original that I wouldn't be able to post updates to CQ&A because CQ&A questions "belong" to the CQ&A team. I'm sure there's a way around it, and I have seen some discussion here, so things may change soon. Until then, I'll keep updating my copy, and I'll update when I can.
