Your tr/// vs. s/// comparison is very broken. You're using the SAME variable in each iteration. You need to do:
transliterate => sub { (my $x = $todo) =~ tr/+/ / },
substitution => sub { (my $x = $todo) =~ s/\+/ /g },
just_copy => sub { (my $x = $todo) },
